Dear Patient:
As you likely already know, the healthcare industry is in the midst of unprecedented change. We want to assure you that your primary care clinic, MRHS has been preparing for this change since we became Nebraska’s first NCQA recognized Patient-Centered Medical Home in 2010. In 2017, we were able to continue and introduce the following services to our patients:
- Focused Disease Days (Diabetic Day, Ladies Health Day)
- Improved Patient Portal services
- Telemedicine/Video appointment types via phone application
- Expanded Clinic hours in morning, noon, and evening
- Behavioral Health Integration
- Chronic Care Management
- Health Living Groups
- “Walk-In” welcome policy
- Addition of new providers
- Induction of Patient Advisory Council (PAC)
- Preventative Health campaigns
As patients, we know that many of you have endured increasing healthcare and insurance costs including a large shift to high-deductible and catastrophic plans. Unfortunately, because of the out-of-pocket expenses related to such plans, many people, maybe even you, are reluctant to see your primary care doctor regularly for disease management, acute issues and/or preventative health. We find this to be true in our practice today. Because we believe that this trend will not result in better health for our patients, we feel compelled to provide a “direct care” option for our patients.
This letter is to introduce to you the MRHS Direct Care Membership Plan. This is not insurance. It essentially allows you unlimited office visits for an affordable and predictable monthly fee. The cost is $75.00 per month per person ($60 for 1st child, and $50 for each addition child). It does not include laboratory, X-ray, immunizations and/or medications. We know unexpected medical expenses can seriously impact your monthly budget. The MRHS Direct Care Plan is a way for you to budget while ensuring access to primary care services.
